New Students – Two Step Registration Process

Help us to plan for your child(ren)'s education.

Step One – Email Pre-Registration form

Please complete the Pre-Registration form and the Release of Records form as soon as you know you will be moving to Jamestown:

Pre-Registration Form: Word 2007 or Word 97-2003

Release of Records Form (if child has previously attended school)

Step Two – Onsite Registration

The following information must be submitted to the Melrose School office prior to starting school:

• Registration Form with immunization series completed (PK-8 Students)
• Application for the Integrated Preschool (Pre-school Students)
• Preschool Tuition Agreement (Pre-school Students)
• Preschool Tuition Payment Options (Pre-school Students)
• High School Form completed (9-12 Students)
• Proof of Residency in Jamestown (copy of utility bill, lease, etc., PK-8)
• Copy of Birth Certificate (PK- 8)
• Home Language Survey
• Grade 6 - Music Selections
• Grade 7 and 8 Music Selections
• Release of Records Form (if child has previously attended school)
• Transportation Form (read Transportation Letter)

The following information must be submitted to the school nurse prior to starting school:

• Complete and current immunization series (see Immunization and Physical Exam Requirements by Grade)
• Current physical examination – within the past six months (form available here)
• Medication form, food/insect allergies form if applicable.
